Be Well: Obesity (Full Program)

>>THE FOLLOWING IS A
PRESENTATION OF BEWELL, A PROJECT PRESENTED BY IT W V I C
PBS IDEASTROM. FUNDING COMES FROM THE DR. DONALD J. GOODMAN FUN, THE MORE
GRIT CAR MORGAN FOUNDATION, THE MACGREGOR FOUNDATION, THE
WOODRUFF FOUNDATION, AND THE COMMUNITY FOUNDATION OF LORAIN
COUNTY.>>THE RATE OF SEVERE OBESITY IN
THE UNITED STATES HAS SKYROCKETED OVER THE LAST
DECADE.>>FAST FOOD IS A LARGE PART OF
THE CULTURE. IT IS OUR LIFESTYLE. IF YOU DON’T TURN IT AROUND, YOU
ARE GOING TO BE OBESE AND OVERWEIGHT.>>WE ARE LITERALLY EATING
OURSELVES TO DEATH.>>I COULD NOT GET AROUND.>>I CANNOT EVEN TIE MY OWN
SHOES.>>I AM ALMOST 50 YEARS OLD AND
I’VE GOT TO LOOK IT WHERE DO WANT TO BE. I’VE GOT TO LOSE SOME WEIGHT.>>WE HAVE LOST FOCUS IN OUR
COUNTRY ON WHAT A NORMAL WEIGHT LOOKS AND FEELS LIKE.>>IT BOILS DOWN TO SAVING
MONEY. OBESITY IS AN EXPENSIVE DISEASE.>>PART OF THE OBESITY EPIDEMIC
IS THAT WE NEED HEALTHY FOOD. WE NEED ACCESS TO IT.>>KIDS IN THIS NEIGHBORHOOD ARE
NOT EATING AS WELL AS THEY SHOULD. THEY ARE EATING A WHOLE LOT OF
JUNK FOOD.>>FAST FOOD STARTED CREEPING
IN. PHYSICAL ACTIVITY BECAME LESS,
AND I WAS A MESS.>>THERE IS NO DOUBT ABOUT IT. THINGS ARE GETTING BIGGER. THE WEIGHT LIMIT FOR AMBULANCES
USED TO BE 800 POUNDS. NOW AMBULANCE CAN ACCOMMODATE
PATIENTS TO WEIGH 1,000 POUNDS OR MORE. THESE DAYS THEY ARE 80
CENTIMETERS WIDE. A STANDARD SIDES CASKET USED TO
BE 24 INCHES WIDE BUT TODAY. 9 INCH WIDE CASKETS ARE IN HIGH
DEMAND. THINGS ARE GETTING BIGGER
BECAUSE WE ARE GETTING BIGGER. OBESITY HAS BECOME A NATIONAL
EPIDEMIC. FOR THE NEXT HOUR YOU’LL HEAR
STORIES FROM LOCAL PEOPLE WHO ARE WAGING THEIR OWN PERSONAL
WARS AGAINST WEIGHT. WE WILL TALK WITH EXPERTS WHO
HELP US LOOK BEYOND THE BOLDEST TO UNDERSTAND THE SCIENCE AND
MEDICINE OF OBESITY. WHAT THE PEOPLE GAIN WEIGHT? WHAT IS AT STAKE FOR ALL OF US
IF WE DON’T STOP THIS DEADLY TREND? AND MAKE NO MISTAKE, IT IS A
TREND. IT STARTED BACK IN THE MID-
1980S. BACK IN 1985 THE TV SHOW MIAMI
VICE WAS A BIG HIT ON NBC. I WAS ON NBC AS WELL, A REPORTER
AT CHANNEL 3, RIGHT HERE IN CLEVELAND. THAT WAS NEARLY 30 YEARS AGO,
AND THEY — YES, I ADMIT, NEARLY 30 POUNDS AGO. IN 1985 THERE WERE EIGHT STATES
WERE 10% OF POPULATION WAS RATED AS OBESE. AS YOU CAN SEE, OHIO WAS ONE OF
THEM. THIS MAP, AND THOSE THAT FOLLOW,
WERE CREATED BY THE CENTERS FOR DISEASE CONTROL TO TRACK THE
PROGRESSION OF THE OBESITY EPIDEMIC. NOW LOOK WHAT HAS HAPPENED IN
JUST FIVE YEARS. IN 1990, THE NUMBER OF STATES
WERE 10% OF THE POPULATION WAS OBESE QUADRUPLED. FIVE YEARS LATER, IN 1995, WE
CHANGE COLOR NO TO DARK BLUE. THAT’S BECAUSE IN THE STATES
COME UP TO 19% OF THE POPULATION WAS OBESE. JUST TWO YEARS LATER, IN 1970 —
IN 1997, BIG TROUBLE. TAN STATES START APPEARING,
REPRESENTING 20% OR HIGHER PREVALENCE OF OBESITY. BY THE YEAR 2000 LOTS OF STATES
ARE TAN AND SUMMER TURNING ORANGE. THAT COLOR INDICATES THAT 25% OF
THE PEOPLE LIVING IN THOSE STATES WERE OBESE. RED STATE START APPEARING IN
2005. THOSE ARE NOT REPUBLICAN STATES,
THOSE ARE STATES WHERE 30% OR MORE OF THE PEOPLE WERE OBESE. FINALLY IN 2010, LAWS OF ORANGE
AND RED, ALL TOLD, A DOZEN STATES WITH THAT OBESITY
PREVALENCE OF 30% OR HIGHER. THINGS ARE PROJECTED TO GET A
WHOLE LOT WORSE OF CURRENT TRENDS CONTINUE. A NEW REPORT BY THE ROBERT WOOD
JOHNSON FOUNDATION INDICATES THAT BY 2030, ALL 50 STATES
COULD HAVE OBESITY RATES OF 44% OR HIGHER AND ONE IN THREE
ADULTS COULD HAVE TYPE 2 DIABETES. I AM JOINED BY DR. MATTHEW KROH,
A SURGEON IN THE DIGESTIVE DISEASE INSTITUTE AT THE
CLEVELAND CLINIC. WELCOME AND THANKS FOR BEING
HERE. SO WHAT HAPPENED? WHY THE DRAMATIC INCREASE IN
OBESITY?>>MANY OF US CONSUME WAY TO
MANY CALORIES. IF YOU FOLLOW THE TYPICAL
AMERICAN LIFESTYLE THERE’S A GOOD CHANCE TO BECOME OBESE. THAT’S EXACTLY WHAT HAPPENED.>>THERE ARE HEALTH CONSEQUENCES
BEYOND JUST BEING BIG. THAT IS THE BURDEN OUR SOCIETY
HAS NOW. THINGS LIKE DIABETES,
CARDIOVASCULAR DISEASE, AND A LOT OF OTHER MEDICAL PROBLEMS
RELATED TO WAIT AND OBESITY. IS NOT JUST A WEIGHT PROBLEM BUT
IT IS BECOMING A MEDICAL BARDEN PARA>>WHEN WE LOOK AT PEOPLE
WHO ARE OBESE, WE SEE DIFFERENT SIZES AND SHAPES.>>DEPENDING ON WHERE THE FAT
IS, IT IS RELATED TO DIFFERENT TYPES OF MEDICAL PROBLEMS. FAT IN THE TORSO AND CHEST AND
ABDOMINAL AREA SEEMS TO BE MORE SINISTER THAN PERIPHERAL FAT. THE FAT AROUND THE TORSO, IT CAN
IMPACT PEOPLE’S LIVE IN TERMS OF OVERALL QUALITY AS WELL.>>WE DON’T SOMEONE WATCHING TO
SAY I FAT IS NOT THERE, I AM OK.>>ALL FAT AND SEVERE
QUANTITIES IS RELATED TO MEDICAL DISEASE.>>IS NOT JUST THE PHYSICAL
ANOMALIES WE WORRY ABOUT, BUT THERE IS AN ECONOMIC IMPACT,
TOO. THERE ARE IMPACTS DOWN THE LINE
BECAUSE OF YOUR SIZE.>>IT RANGES FROM SOCIAL
DISTURBANCES. AS A SURGEON AND A PHYSICIAN, WE
ARE LOOKING AT THE MEDICAL BOARD OF THIS. CHRONIC, LONG-TERM DISEASE IN
TERMS OF MORTALITY AND MORBIDITY, HIGH BLOOD PRESSURE,
ALL RELATED TO OBESITY.>>WHEN YOU TALK AMONG
YOURSELVES AS DOCTORS, ARE YOU AFRAID OF FOR THE COUNTRY’S
GOING RIGHT NOW?>>I THINK SO. WE HAVE MUSCLE WHOLE SLEW OF
PHYSICIANS LOOKING AT IS A MEDIEVAL — AT A DEEPER LEVEL. I THINK WE ARE ADDRESSING THE
PROBLEM.>>TALK TO ME ABOUT THE FUTURE. 20 YEARS DOWN THE LINE, DO YOU
SEE US HAVING TURNED THE CORNER, OR DO YOU SEE IT GETTING WORSE?>>THE CONSEQUENCES ARE
SIGNIFICANT. WE HAVE A MORE SIGNIFICANT —
MORE SOPHISTICATED UNDERSTANDING OF WHAT GOES INTO OBESITY AND
THE MEDICAL PROBLEMS WITH IT. WE NEED A MULTI BROAD APPROACH
TO EFFECTIVELY DEAL WITH THIS PROBLEM>>CAN WE TURN THE
CORNER?>>I THINK WE CAN. I THINK CHRONIC DISEASES LIKE
DIABETES AND HEART DISEASE RELATED TO OBESITY, WE ARE
UNDERSTANDING THE MECHANISMS WITH THEM.>>DR. MATTHEW KROH OF THE
CLEVELAND CLINIC, THANK YOU FOR COMING IN. MANY OF US CONSUME WAY TOO MANY
CALORIES. FAST FOOD, PROCESSED UP THAT IS
FULL OF SUGAR. TO TOP IT OFF, MANY OF US SUPER
SIZE IT. YOU MIX THAT WITH OUR MODERN,
SEDENTARY LIFESTYLE, AND THAT SPELLS TROUBLE. IF YOU FOLLOW THE TYPICAL
AMERICAN LIFESTYLE, THERE IS A GOOD CHANCE YOU WILL BECOME
OBESE. THAT IS EXACTLY WHAT HAPPENED
TO 48-YEAR-OLD RANDY CARTY. THIS DAY OF RECKONING CAME AT A
MOST UNUSUAL SPOT.>>I HAVE ALWAYS BEEN AN ACTIVE
GUY AND I’VE ALWAYS ENJOYED AMUSEMENT PARKS. WHO WOULD HAVE THOUGHT A VISIT
TO AN AMUSEMENT PARK WOULD BE THE TURNING POINT, THE EYE
OPENER UH WHERE I DECIDED TO TURN MY LIFE AROUND.>>BUT YOU WERE NOT ALWAYS
OVERWEIGHT. TELL ME ABOUT YOU AS A CHILD AND
IN HIGH SCHOOL.>>I WAS A VERY THIN, SMALL
CHILD. I GRADUATED FROM HIGH SCHOOL
WEIGHING ABOUT 150 POUNDS.>>RANDY LISTED IN THE ARMY AND
SERVED AS CREW CHIEF FOR HUEY AND BLACK HAWK HELICOPTERS. THE ARMY KEPT HIM IN GREAT
SHAPE. COULD YOU RUN, YOU PHYSICALLY
TRAIN, YOU ARE PHYSICALLY ACTIVE.>>SO WHAT HAPPENED AFTER THE
ARMY?>>I STARTED WORKING AND GOING
TO SCHOOL TO BECOME A RESPIRATORY THERAPIST AND
BURNING THE CANDLE AT BOTH ENDS, WORKING AND GOING TO SCHOOL. THE DIET STARTED SLIDING, THE
FAST FOOD STARTED CREEPING IN, THE PHYSICAL ACTIVITY BECAME
LESS.>>BRANDIES HOBBY WITH CARS
ADDED FUEL TO THE FIRE.>>I AM A HOT ROTOR AND CLASSIC
CAR GUY. I GO TO A LOT OF TRACKS, I
RACE, I GO TO CAR CRUISES, AND WHAT DO THEY SERVE THERE? BERBERS, DOGS, FRIES, SHAKES,
WHICH AT THAT POINT AS A SINGLE MINE BECAME — AS A SINGLE MAN,
BECAME THE MAINSTAY OF MY DIET. IT SNEAKS UP ON YOU. YOU LOOK IN THE MIRROR AND YOU
DON’T REALIZE YOU ARE POUNDS, AND BEFORE YOU KNOW IT, YOU ARE
350, 360 POUNDS. I GOT MARRIED 3 YEARS AGO. MY WIFE KEPT TELLING ME, YOU ARE
ALWAYS A HANDSOME MAN TO ME, BUT YOU NEED TO TURN THIS AROUND A
LITTLE BIT. I HAD NO PERSON TO BLAME BUT
MYSELF. I KNEW BETTER. I HAD BEEN IN THE MILITARY, A
CRITICAL CARE CLINICIAN OF 18 YEARS. I JUST LET IT GO.>>AND YOU WENT WITH THE FLOW.>>I WENT WITH THE FLOW. THERE’S NO ONE TO BLAME BUT
MYSELF. I DID IT MYSELF. SLOWLY BUT SURELY. MY SHIRTS WENT FROM EXTRA LARGE
TO XX AND EVENTUALLY XXX.>>TAKE THIS BACK TO THAT DATE,
WHAT HAPPENED?>>I WANTED TO RIDE THE RAPTURE
AND HAVE A SHOULDER RESTRAINT THAT COMES DOWN ACROSS TO. MUCH TO MY AMAZEMENT AND SHOCK,
I WAS 5 OR 6 INCHES AWAY FROM EVEN BUCKLING IN. IT WAS EXTREMELY EMBARRASSING,
AND I GOT FRUSTRATED AND I THREW THE THING OFF MY SHOULDERS, AND
I GOT OFF THE RIDE AND STOOD ON THE PLATFORM WITH MY HEAD
HANGING LOW IN DISBELIEF THAT I CANNOT FIT ON A ROLLER COASTER. I AM TOO FAT TO FIT ON A ROLLER
COASTER, OH MY GOD. I REMEMBER FEELING SO DEPRESSED
AND UPSET ABOUT THIS. I WENT OVER TO A HAMBURGER
JOINT AND I ORDERED A DOUBLE CHEESEBURGER, FRIES, AND A
SHAKE, AND I SAT DOWN AND I BEGAN EATING IT. I THOUGHT WELL, IF I CANNOT RIDE
THE RIDES, I AM GOING TO HAVE SOME GOOD FOOD.>>ON VALENTINE’S DAY OF 2012,
RANDY GOT ANOTHER WAKE-UP CALL. MEDICAL TESTS REVEALED HE HAD
DIABETES AND LOTS OF OTHER PROBLEMS.>>I WAS A MESS. MY BODY WAS GOING TO BLOW. I HAD 6 OF THE 8 RISKS FOR
STROKE AND HEART ATTACK STARING ME IN THE FACE, AND I AM A
NONSMOKER.>>RANDI DECIDED TO HAVE GASTRIC
BYPASS SURGERY AT THE CLEVELAND CLINIC, A PROCEDURE THAT
BASICALLY RETOOLS YOUR INSIDE SO YOUR STOMACH SHRINKS. RANDY CREDITS IT WITH SENDING
HIS DIABETES INTO REMISSION.>>I KNEW THAT AFTER THE
PROCEDURE I HAD TO DRASTICALLY MAKE A CHANGE, EATING HABITS,
LIFESTYLE, EXERCISE, PROPER NUTRITION WAS PARAMOUNT. YOU DISCOVER QUICKLY OR WITHIN
THE FIRST FEW MONTHS WHAT YOU CAN AND CANNOT EAT. NO CARBONATED BEVERAGES, NO
BEER, NO RED MEAT, NO FRIED FOOD. SOMETHING CLICKS AND SNAPS WHEN
YOU HAVE THE PROCEDURE. I LOVE SEAFOOD NOW.>>YOU WENT FROM A 44 WAIST TO A
32?>>32 IS THE NEW ME.>>RANDY NOW WEIGHS ABOUT 190
POUNDS. BUT THE REAL PAYOFF CAME BACK AT
CEDAR POINT WHEN HE RETURNED TO RIDE THE RAPTOR WITH HIS WIFE,
AND THE SHOULDER HARNESS FIT.>>IT FIT. I WAS HAPPY. I LOOKED AT HER AND SAID, IT
FITS. IT WAS A GOOD DAY.>>TWO MONTHS AFTER RANDY
CERTIFICATION, HIS WIFE OPTED TO HAVE THE SAME PROCEDURE. IT IS AN OPTION A LOT OF PEOPLE
ARE CHOOSING THESE DAYS. JOINING ME NOW IS DR. AVIV BEN-
MEIR. WE WILL DIVE DEEPER INTO
BARIATRIC SURGERY IN JUST A MOMENT. THE SEE MANY PATIENTS WHO BECAME
OBESE BECAUSE OF LIFESTYLE CHOICES THEY MAKE?>>I DON’T KNOW IF IT IS FAIR TO
SAY IT QUITE THAT WAY. THERE ARE A LOT OF FACTORS THAT
GO INTO PEOPLE GAIN WEIGHT. PART OF IT IS AS SIMPLE AS SOME
OF US REQUIRE FEWER CALORIES THAN OTHER INDIVIDUALS. A COUPLE OF HUNDRED YEARS AGO,
WE WERE WORKING IN THE FIELDS. WE WERE FARMERS. WE COULD EAT MORE CALORIES
BECAUSE WE WERE EXPANDING THEM. NOWADAYS, WE HAVE A SEDENTARY
LIFESTYLE. WE HAVE COMPUTER-BASED JOBS. WE HAVE READILY AVAILABLE
PROCESSED FOODS, HIGH CALORIE, NON NUTRITIOUS FOODS. I THINK ALL THOSE THINGS COME
TOGETHER. IT IS NOT JUST NOT EXERCISING OR
MAKING POOR CHOICES.>>WHEN WE LOOK AT RANDY STORY,
IS HE A TYPICAL OR GOOD CANDIDATE FOR BARIATRIC SURGERY?>>I THINK RANDY OBVIOUSLY WAS
AN EXCELLENT CANDIDATE FOR BARIATRIC SURGERY. HE WAS 350, 360 POUNDS. HE WAS DIABETIC. HE HAD MULTIPLE RISK FACTORS
FOR HEART DISEASE. THE DOCTOR BILLED HIM A GASTRIC
BYPASS. HE IS USING IT CORRECTLY AND HE
HAS LOST WEIGHT.>>TELL ME ABOUT HOW THE SURGERY
ACTUALLY WORKS. PEOPLE DON’T KNOW EXACTLY WHAT
HAPPENS THERE.>>THERE ARE SEVERAL OPERATIONS
THAT ARE AVAILABLE NOWADAYS TO HELP PEOPLE LOSE WEIGHT. WITH GASTRIC BYPASS, WE SIMPLY
HELP THE PATIENT FEEL FULL WITH LESS FOOD. OUR STOMACHS ARE THE SIZE OF A
FOOTBALL. HIS STOMACH WAS CONVERTED INTO
TWO PARTS. ONE THE SIZE OF MY THUMB, AND
THE REST OF THE STOMACH IS BYPASSED. NOW RANDY FEELS FULL WITH A
SMALLER AMOUNT OF FOOD.>>SOMEBODY OUT THERE JUST SAID,
THE SIZE OF YOUR THUMB, REALLY?>>OUR STOMACHS ARE THE SIZE OF
A FOOTBALL AND THAT CAN MAKE IT DIFFICULT TO FEEL FULL FOR A
NORMAL SIZE PERSON. WHEN WE PERFORM A GASTRIC
BYPASS, WILL LITERALLY MAKE THE PARTY THAT FEELS FOOD THE SIZE
OF MY THUMB.>>WHAT OTHER MAIN ITS DOES HE
HAVE TO DO TO KEEP THE WEIGHT PARTY IS NOW?>>HASTERT BYPASSES JUST A TOOL. FOR IT TO BE SUCCESSFUL, THERE
IS AN OWNER’S MANUAL INVOLVES WHEN YOU EAT, HAVING A LIFESTYLE
CHANGE. YOU FEEL FULL WITHIN A SMALL
AMOUNT OF TIME. IT REVOLVES AROUND THINGS LIKE
KEEPING THE MEALS SHORT, 20-30 MINUTES, NOT NIBBLING ALL DAY
LONG. NOT DRINKING WITH YOUR MEALS. DRINKING WITH YOUR MEALS CLASHES
THE FOOD OUT OF THAT BOMB SIZE POUCH, SO YOU CAN EAT MORE. NOT GETTING YOUR CALORIES FROM
LIQUID, AND MAKING SURE YOU HAVE PROTEIN WITH EVERY MEAL.>>DOES INSURANCE COVER THIS? I CANNOT IMAGINE THIS IS AN
INEXPENSIVE OPERATION.>>NO, IT IS NOT INEXPENSIVE. MOST PATIENTS HAVE THEIR
INSURANCE COMPANY COVERAGE. NOT EVERY IN — NOT EVERY
INSURANCE COMPANY COVERS IT FOR EVERY POLICY. THERE MAY BE AN EXCLUSION.>>TO SOME PEOPLE CONSIDER IT A
LAZY MAN’S WAY OF LOSING WEIGHT? I WILL JUST SHRINK MY STOMACH.>>I WOULD SAY THAT A LOT OF
PEOPLE THINK THAT. I THINK THAT AS PART OF THAT
THIS — DISCRIMINATION IN ITS MORBID OBESITY. THIS IS NOT A LAZY MAN’S WAY TO
LOSE WEIGHT. THIS IS A CHOICE THAT PATIENTS
MAKE, ONCE THEY FEEL THAT THEY CANNOT LOSE THE WEIGHT AND KEEP
IT OFF WITHOUT OUR HELP.>>IS THERE ANY POINT WHERE WE
SEE A LOT OF PEOPLE REGRESSING, GOING BACK TO THEIR WEIGHT, OR
IS IT A PERMANENT FIX?>>IT IS A PERMANENT POOL, BUT
IS IT A PERMANENT FIX? FOR SOMEONE TO BE SUCCESSFUL, I
HAVE TO DO A GOOD JOB BUILDING THAT TOOL. THE PATIENTS HAVE TO BE TAUGHT
HOW TO USE THAT TOOL, BUT THEN THEY HAVE TO USE IT.>>THANK YOU SO MUCH FOR COMING
IN.>>THANK YOU FOR HAVING ME.>>MEDICINE IS A NUMBERS GAME. DOCTORS USE ALL SORTS OF NUMBERS
THAT ACT AS BENCHMARK MEASUREMENTS TO DETERMINE HOW
HEALTHY WE ARE. GOOD VISION IS 20-20. A NORMAL TEMPERATURE IS 98.6. YOUR BLOOD PRESSURE SHOULD BE
120 OVER 60. ALL WE MAY NOT UNDERSTAND
EXACTLY WHAT THESE NUMBERS MEAN, WE EASILY KNOW WHAT NORMAL OR
GOOD IS. BUT WHAT ABOUT OUR WAY? 180 POUNDS MAY SOUND GOOD, BUT
NOT IF YOU ARE 5 FEET TALL. HAVE YOU KNOW IF YOU ARE FAT OR
FIT? THE NUMBER DOCTORS USED TO
CATEGORIZE YOUR WEIGHT IS CALLED BMI, OR BODY MASS INDEX.>>BODY MASS INDEX IS A FORMULA
THAT USES A RATIO OF WAY TO HIDE TO COME UP WITH A NUMBER. ANYTHING 30 OR ABOVE IS
CONSIDERED OBESE. THE FORMULA WAS CREATED IN 1835
BY A BELGIAN STATISTICIAN. HUNDREDS OF YEARS LATER, IT
STANDS AS A SIMPLE TOOL THAT HELPS DOCTORS’ FACTOR YOUR FAT.>>MANY PEOPLE REALIZE THEY ARE
OVERWEIGHT, BUT SHOWING THEM THE BMI GIVES US AN IDEA AND GIVES
THE PATIENT AN IDEA OF HOW SEVERE THE PROBLEM IS.>>MOVE BACK AND STAND STRAIGHT
UP FOR ME.>>MANY DOCTORS USE OF BMI CHART
TO PINPOINT YOUR WEIGHT CATEGORY. YOU CAN FIND OF BMI CALCULATOR
ON THE INTERNET. YOU WILL FIND ABOUT 5 MILLION
RESULTS ON GOOGLE. BMI IS IMPORTANT BECAUSE IT IS
AN EASY TOOL THAT HELPS DOCTORS ASSESS YOUR RISK FOR KILLER
DISEASES.>>PEOPLE NEED TO KNOW THEIR
BIOMETRICS. BMI, THEIR BLOOD PRESSURE,
CHOLESTEROL, BLOOD SUGARS, THESE ARE ALL IMPORTANT NUMBERS TO
HELP US CLASSIFY PATIENTS RISKS FOR CERTAIN DISORDERS,
PARTICULARLY CARDIOVASCULAR DISEASE AND BOUGHT — AND
DIABETES. X BUT BE MY DOES NOT WORK FOR
EVERYONE. IT DOES NOT BREAK DOWN HOW MUCH
OF YOUR BODY IS COMPOSED OF FAT VERSUS MUSCLE. WADE — WAIST SIZE IS ANOTHER
IMPORTANT MEASURE. NOT ALL FAT IS CREATED EQUAL. THE KIND THAT SINCE THE BENEATH
YOUR BELLY IS THE MOST DANGEROUS BECAUSE IT CAN HARM NEARBY
ORGANS LIKE THE PANCREAS AND LIVER. THERE ARE OTHER HIGH TECH WAYS
TO MEASURE FAT LIKE USING AIR PRESSURE IN SPECIALIZE CHAMBERS. NO MATTER HOW YOU MEASURE IT,
ONE THING IS FOR SURE. OBESITY IS ASSOCIATED WITH LOTS
OF DIFFERENT MEDICAL PROBLEMS.>>IT IS ON THE CAUSAL PATHWAY
TO EVERY MAJOR CHRONIC DISEASE THAT PLAGUES OUR SOCIETY, HEART
DISEASE, CANCER, A STROKE.>>PEOPLE WITH CLINICALLY SEVERE
OBESITY HAVE A BODY MASS INDEX OF MORE THAN 40. IN THE LAST DECADE, THE RATE OF
SEVERE OBESITY HAS SKYROCKETED. YOU MIGHT BE THINKING, HOW CAN
PEOPLE GET THAT HEAVY? ONE REASON LIES DEEP WITHIN OUR
DNA. OBESITY TENDS TO RUN IN
FAMILIES. SCIENTISTS THINK THEY ARE A IS A
COMBINATION OF GENETIC FACTORS AT WORK. ANOTHER FACTOR COMES INTO PLAY,
FAMILY CULTURE. CERTAIN ETHNIC AND RACIAL GROUPS
TEND TO EAT LOTS OF HIGH CARBON AND HIGH FAT FOODS. IN OUR NEXT STORY, WE MEET 56-
WEROSIE NAGY.>>WE ALWAYS GET TOGETHER AT
LEAST ONCE A WEEK. MY BROTHER COMES OVER AND WE
WILL PLAY CARDS. WE HAVE ALWAYS BEEN A CLOSE KNIT
FAMILY. FOOD WAS THE CENTERPIECE OF WHAT
WENT ON IN OUR LIVES. WHEN WE GATHERED, THERE WAS
ALWAYS FOOD ON THE TABLE. THAT WAS WHAT WE DID, WE ATE. THAT WAS PART OF OUR CULTURE. MY MOTHER’S SIDE OF THE FAMILY
WAS ITALIAN. MY GRANDMOTHER, WE ALWAYS CALLED
HER THE FOOD PUSHER. YOU HAVE TO EAT, WHAT DO YOU
MEAN YOU CAN’T EAT? SHE MADE MEAT DISHES WITH HEAVY
LADEN SAUCES, THE CHEESES. ON MY DAD’S SIDE, HE WAS SLOVAK. THE THE HOLIDAYS, THERE WAS JUST
WAY TOO MUCH FOOD. BOTH OF MY GRANDMOTHERS, MY DAD,
MY MOM WAS HEAVY AT ONE TIME. MY BROTHER WAS HEAVY. AND A LOT OF MY AUNTS, MY MOM’S
SISTERS, THEY GOT HEAVY OVER THE YEARS. BUT WE DIDN’T SEE OURSELVES AS
HEAVY. WE FELT WE WERE NORMAL. I HAVE HAD A WEIGHT PROBLEM
PRETTY MUCH MY WHOLE LIFE. I WAS BORN A BIG BABY, CLOSE TO
10 POUNDS WHEN I WAS BORN. IT WAS HARD GROWING UP. FOR THE MOST PART, MOST PEOPLE
MADE FUN OF ME. NEVER HAD A NICE THING TO SAY. SO IT WASN’T A FUN EXPERIENCE TO
GO TO SCHOOL. I TRIED MANY TIMES TRYING TO GET
OUT OF SCHOOL, FAKED BEING SICK. IT’S ALMOST LIKE WHAT BULLYING
IS TODAY. YOU JUST DIDN’T WANT TO GO. PROBABLY IN MY MID-FORTIES, I
BECAME MY HEAVIEST. I WAS STARTING TO EXPERIENCE
SOME HEALTH ISSUES. I COULD NOT GET AROUND. I WAS 526 POUNDS. THERE WERE A LOT OF THINGS I
COULDN’T DO. IT MAKES ME EMOTIONAL. IT WAS VERY HARD BECAUSE MY
FAMILY HAD TO DO EVERYTHING FOR ME. I COULDN’T EVEN TIE MY OWN
SHOES. I COULDN’T GET AROUND. I COULDN’T WORK. PEOPLE ARE VERY JUDGMENTAL. THEY DON’T WANT TO GET TO KNOW
YOU. I AM A PERSON. IT DOESN’T MATTER WHAT I LOOK
LIKE. I WAS MORE EMBARRASSED THAN
ANYTHING. BUT THEY WERE MY FAMILY AND THEY
WERE ALWAYS THERE FOR ME. I AM SORRY. AND I TRIED ALL THE BOOK DIETS. NO EATING. NONE OF THEM WORKED. I MEAN, THEY WORK FOR A WHILE,
BUT THEY ARE NOT TEACHING YOU THE PROPER WAY TO EAT. SO I WASN’T LEARNING ANYTHING. I WAS DOING MORE DAMAGE TO ME,
MY SYSTEM. I NEEDED TO LEARN HOW TO EAT
SENSIBLY, EAT THE THINGS I WANTED TO EAT, AND STILL LOSE
WEIGHT AND CONTINUE ON THAT. IT BECOMES OVERWHELMING. YOU DON’T THINK YOU ARE EVER
GOING TO BE ABLE TO TAKE IT OFF. I WILL BE HEAVY THE REST OF MY
LIFE.>>FINALLY, ONE DAY AT A WEIGHT
WATCHERS MEETING, ROSIE GOD INSPIRED. SHE WAS DETERMINED, AND THE
WHITE STARTED COMING OFF. BUT THEN IN 2010, ROSIE WAS
DIAGNOSED WITH YOU DURING CANCER AND HAD A HYSTERECTOMY. SHE LEARNED LESS WEIGHT AND MORE
EXERCISE COULD PREVENT A RECURRENCE. SO ROSIE JOINED A HEALTH CLUB. IT WAS THE FIRST TIME SHE HAD
EVER SET FOOT IN A GYM. WHAT DO YOU REMEMBER ABOUT THE
FIRST DAY WHEN YOU CAME TO WORK OUT?>>I WAS NERVOUS. I JUST DIDN’T BELIEVE THAT I
COULD BE HERE AND DO ANYTHING BECAUSE I WAS SO OVERWEIGHT. I COULDN’T DO MUCH.>>AND SLOWLY BUT SURELY YOU GOT
INTO IT.>>I DID GET INTO IT. WHEN I THINK OF MY FIRST DAY
HERE AND WHERE I AM TODAY, I NEVER THOUGHT I WOULD GET THERE,
BUT I AM. I AM THERE.>>LOSING WEIGHT HAS BECOME A
FAMILY AFFAIR. HER MOM LOST 50 POUNDS AND HER
BROTHER HAS LOST 100.>>TRY LOSING IT AND NOT GAINING
IT BACK. THAT IS THE KEY. MY MOM, MY SISTER, THEY HAVE
WORKED VERY HARD AT THIS. WE STILL LIKE TO EAT, BUT NOW IT
IS LIKE ABOUT MAKING THE RIGHT CHOICE.>>NOW AT FAMILY GATHERINGS,
TRADITIONAL LASAGNA IS REPLACED WITH THE VEGETARIAN VERSION. SO FAR, ROSIE HAS LOST 230
POUNDS.>>THIS IS THE HAPPIEST I HAVE
BEEN IN MY LIFE. I HAVE A LOT OF FRIENDS THAT ARE
TRUE FRIENDS. THEY SEE ME AS ROSIE, NOT
SOMEBODY THAT IS STRUGGLING WITH THEIR WEIGHT, OR HEAVY. EVEN THOUGH I AM STILL HEAVY,
PEOPLE DON’T LOOK AT ME LIKE, YOU KNOW, WHO’S THAT, OR WE
DON’T WANT TO TALK TO YOU.>>GO ALL THE WAY UP AND THEN
BRING IT DOWN AGAIN. YOU SHOULD BE FEELING THAT RIGHT
THROUGH THERE.>>AS YOU GO ON THROUGH LIFE,
YOU HAVE TO MAKE CHOICES. I KNOW IT’S NOT A LIFE SENTENCE,
BUT I HAVE TO BE THE ONE TO CHANGE IT.>>ROSIE IS A CLASSIC EXAMPLE OF
SOMEONE WITH A GENETIC PREDISPOSITION TO OBESITY. HER CASE IS COMPOUNDED BY THE
FACT THAT FOOD AND EATING IS SUCH A BIG PART OF WORK FAMILY
CULTURE. DR. EILEEN SEEHOLZER JOINS US
NOW. SUCH AN EMOTIONAL STORY. THERE IS THE DNA YOU COME WITH,
AND THEN THERE IS THE CULTURE, THE ETHNICITY. TWO VERY DIFFERENT PROBLEMS TO
DEAL WITH.>>THEY ARE, BUT THEY ARE ALSO
INTERTWINED. IT LOOKS LIKE ABOUT 40-70% OF
OUR VULNERABILITY TO BECOME OBESE IS DETERMINED BY GENES. THAT BEING SAID, WE HAVE THE
SAME GENES WE HAD 400 YEARS AGO WHEN WE DID NOT HAVE THE OBESITY
EPIDEMIC THAT WE HAVE. THE ENVIRONMENT IS THE THING
THAT PULLS THE TRIGGER IN SOME CASES AS TO WHETHER OBESITY
DEVELOPS OR NOT.>>ROSIE WAS ALWAYS BIG. THAT PLAYS A BIG PART IN IT, THE
FACT THAT SHE WAS A BIG KID, SHE IS GOING TO BE A BIG ADULT.>>WE DO KNOW THAT KIDS WHO
STRUGGLE WITH OBESITY, UNFORTUNATELY ALMOST ALWAYS DO
BECOME OBESE ADULTS. THAT IS PROBLEMATIC FOR A COUPLE
OF REASONS. SHE STRUGGLED A LOT WITH THE
STIGMA THAT OBESITY COST HER SOCIALLY, GROWING UP. SHE TALKS ABOUT HOW DIFFICULT IT
WAS TO GO TO SCHOOL AND TO FEEL GOOD ABOUT HERSELF. THE SECOND THING AS WELL AS
THOSE HEALTHY HABITS WITH EATING AND ACTIVITY ARE LESS LIKELY TO
BE LEARNED AS A KID BECAUSE YOU ARE IN VERNON DID NOT HAPPEN TO
INCLUDE THOSE THINGS.>>SHE MENTIONED THAT HER
BROTHER WAS HEAVY, TOO.>>IN ADDITION TO THE HOME
ENVIRONMENT, IN ADDITION TO THE GENES WE TALKED ABOUT THAT EXIST
WHEN WE ARE CONCEIVED, THE ENVIRONMENT FOR A BABY ACTUALLY
STARTS IN THE MOMS UTERUS WHEN THEY ARE A FETUS. IT TURNS OUT THAT AS A MOM GETS
HEAVIER HERSELF, THE ENVIRONMENT FOR THE BABY CHANGES SO THE BABY
HAS A HIGHER LIKELIHOOD OF BECOMING OBESE OR HAVING A HIGH
BIRTH WEIGHT AND DEVELOPING DISEASES RELATED TO THAT LATER. SHE MENTIONED SHE WAS ABOUT 10
POUNDS WHEN SHE WAS BORN, AND THAT IS COMMONLY ASSOCIATED WITH
MOMS WHO ARE HEAVIER AS WELL.>>IT IS DIFFICULT FOR PEOPLE
LIKE ROSIE TO TRY AND DO WELL AND EAT WELL WHEN ALL THE PEOPLE
AROUND YOU ARE NOT DOING WELL, NOT EXERCISING AND EATING WELL.>>IT HAS A LOT TO DO WITH THEIR
ENVIRONMENT AND THEIR OPPORTUNITY, AND THE GENES WE
HAVE HAVE A LOT TO DO WITHOUT QUICK WE STOP EATING AND HOW
HUNGRY AND HOW MUCH WE CRAVE THINGS. THAT ENVIRONMENT IS VERY KEY. IF YOU HAVE THAT GENETIC
TENDENCY, AND THE CURRENT TERM AND IS ONE WHERE FOOD IS ALWAYS
PRESENT AND PREVALENT, WITH EMOTIONAL EVENTS ON TOP OF IT,
SHE REFERRED TO HER GRANDMOTHER AS A FOOD PUSHER. YOU LEARN TO EAT WHEN YOU ARE
NOT HUNGRY AND AVOID SOME OF YOUR BODY’S NATURAL CUES.>>WHEN SOMEBODY DEVELOPS
OBESITY TO A CERTAIN SEVERITY, THE FAT CELLS BURST EXPAND BUT
ACTUALLY MULTIPLIED OVER TIME. THE SHORT ANSWER IS YES, BUT AS
SHE LOSES THIS WAY, AND SHE IS DOING EXACTLY WHAT WE HOPE
PEOPLE DO, WHICH IS CHANGE HER APPROACH AND LIFESTYLE. SHE IS EATING DIFFERENTLY EVERY
DAY. SHE IS MOVING MORE EVERYDAY. THE THING FOR HER IS, IF SHE
STOPS THAT, SHE WILL REGAIN. ON TOP OF THAT, HER ABILITY TO
GET THE WEIGHT OFF IS LIMITED BY HOW MUCH WEIGHT SHE HAD BEFORE. BASICALLY THOSE THAT CELLS THAT
ARE STILL PRESENT AND THAT HAVE MULTIPLIED MAKE IT TOUGHER FOR
HER TO ACCOMPLISH WHAT SHE IS ACCOMPLISHING. HER WORK IS TO BE MORE ADMIRED
BECAUSE OF THE CHALLENGES SHE HAS FROM INSIDE AS WELL AS OUT.>>HOW DAMAGING IS THE YO-YO
EFFECT TO THE BODY?>>THERE IS A LOT OF CONFLICTING
DATA ON THAT. THE BIGGER ISSUE FOR A LOT OF
COMMERCIAL DIETS ARE DIFFERENT WEIGHT LOSS SYSTEMS HAS TO DO
WITH NOT HAVING A PRACTICAL WAY TO ADOPT A HEALTHIER WAY OF
EATING LONG TERM. PEOPLE COME FOR 12 WEEKS AND
THEY CANNOT SUSTAIN IT AND THEY JUST GO BACK TO WHERE THEY WERE
AND THEY DON’T LEARN ANYTHING. SHE IS DOING MORE AND MORE
PROGRESSIVELY THROUGH A LIFETIME. THAT IS REALLY THE ONLY ROAD TO
IMPROVEMENT.>>SO THE BOTTOM LINE IS THERE
IS HOPE?>>THERE IS ABSOLUTELY HOPE. CULTURALLY IT IS MORE
ACCEPTABLE TO SAY I REALLY NEED TO MAKE BETTER CHOICES.>>THANK YOU SO MUCH. THIS PROGRAM ABOUT OBESITY IS
JUST ONE PART OF OUR ONGOING MULTIPLE MEDIA HELP INFORMATION
SERIES CALLED BE WELL. OBESITY IS SUCH AN IMPORTANT
TOPIC THAT WE INTEND TO BRING YOU MORE RADIO, TELEVISION, AND
WEB CONTENT FOR MUCH OF THIS YEAR. ARTICLES, STUDIES, STORIES AND
LOCAL RESOURCES ARE ALL DESIGNED TO HELP YOU BETTER UNDERSTAND
OBESITY IN HOW YOU AND YOUR FAMILY CAN LIVE HEALTHIER LIVES. BE SURE TO WATCH THE WEB
EXCLUSIVE INTERVIEW — INTERVIEW I DID WITH DR. WILLIAM DIETZ, A
NATIONALLY RESPECTED OBESITY EXPERT WHO WAS IN CLEVELAND FOR
THE TOWN HALL SPEAKER SERIES. YOU CAN ALSO WATCH HIM INTERACT
LOCAL SCHOOL STUDENTS WHO PORTIS UPDATED IN A DISTANCE LEARNING
EDUCATION PROGRAM. ALL THIS AND MUCH MORE WHEN YOU
GO TO HEALTH.IDEASTREAM.ORG. ANOTHER BECAUSE OF OBESITY STEMS
FROM STRESS. A PROLONGED STRESS CAN CAUSE
YOUR BODY TO UNLEASH A A HORMONE CALLED CORTISOL THAT INCREASES
YOUR APPETITE. IN SOME CASES CHRONIC STRESS CAN
ACTUALLY CAUSE THE BODY STRESS RESPONSE SYSTEM TO GET STUCK IN
THE ON POSITION, CAUSING PERSISTENT HORMONAL PROBLEMS
THAT LEAD TO DIABETES AND OTHER COMPLICATIONS. ALL THIS IS MAGNIFIED BY THE
HIGH LEVELS OF SUGAR, FAT, AND SALT AND FAST FOODS. DAVID RUTAN CAN TELL YOU
SOMETHING ABOUT BUSY SCHEDULES. STRESS AND HOW THEY INCREASE OUR
CRAVINGS FOR FAST AND EASY COMFORT FOOD.>>MY NAME IS DAVID RUTAN. I AM 48. I AM A HUSBAND, FATHER, AND I
HAVE TWO CHILDREN. WE ARE THE TYPICAL AMERICAN
FAMILY OF BOTH PARENTS WORKING, HAVING CAREERS, HAVING CHILDREN
AND THE CHILDREN BEING VERY ACTIVE. I GRADUATED IN 1986 FROM THE
UNIVERSITY OF AKRON. I HAVE A DEGREE IN COMPUTER
SCIENCE. I WENT BACK TO SCHOOL AND GOT A
MASTER’S DEGREE IN INFORMATION SECURITY. THERE ARE A LOT OF DEADLINES, A
LOT OF STRESS. PEOPLE NEED THINGS THAT I HAVE
TO GET DONE FOR THEM.>>WHEN DAVID AND HIS WIFE
STARTED THEIR FAMILY, WHITE WAS NOT REALLY AN ISSUE. BUT AS THEIR KIDS GOT OLDER AND
MORE INVOLVED IN SPORTS, THINGS BEGAN TO CHANGE.>>MY DAUGHTER STARTED FENCING
ABOUT 7 YEARS AGO.>>YOU HAVE LITERALLY SPENT
HOURS HERE AT THE FENCING STUDIO.>>WE TRAVEL DOWN THREE OR FOUR
NIGHTS A WEEK AND SPEND TWO OR THREE HOURS A NIGHT. WHEN MY DAUGHTER WAS NOT
FENCING, MY SON WAS PLAYING BASEBALL SOMEWHERE. WHEN HE WAS WITH A CLUB TEAM, HE
HAD PRACTICE 3 OR 4 NIGHTS A WEEK, AND MY DAUGHTER HAD
PRACTICE 3 OR 4 NIGHTS A WEEK. IT WAS CONSTANTLY GET IN THE
CAR, DROP ONE OFF, DRIVE BACK, PICK THE OTHER ONE UP, DRIVE
BACK, GET THE FIRST ONE YOU DROPPED OFF.>>DAVID IS PART OF THE SANDWICH
GENERATION, BALANCING KIDS, CAREER, AND AGING PARENTS.>>BOTH OF OUR MOTHERS HAVE
PASSED AWAY. BOTH OF US HAVE AGING FATHERS. THE STRESS IS THE RESPONSIBILITY
TO MAKE SURE IF THEY HAVE WHAT THEY NEED. I THINK SOMETIMES YOU KIND OF
LOOSE FOCUS ON TAKING CARE OF YOURSELF VERSUS TAKING CARE OF
THE OTHER PEOPLE AROUND YOU. THEY BECOME YOUR PRIORITY AND
YOU START DEVELOPING BAD HABITS.>>HABITS LIKE NOT MAKING TIME
TO EXERCISE OR RELAX. GOOD NUTRITION TAKES A BACK SEAT
WHEN THERE IS BARELY TIME TO EAT.>>THE MORE STRESS YOU FEEL, THE
WORSE YOU EAT, AND IT SNOWBALLS A LITTLE BIT. YOU GO THROUGH A CYCLE OF YOU
JUST CANNOT GET OUT OF THAT. YOU CRAVE OF FRIES AND STARCHES
AND PROCESSED SUGARS.>>FOR DAVID, THE TIPPING POINT
CAME WHEN HE HIT 300 POUNDS. HIS BLOOD PRESSURE WAS UP AND SO
WAS HIS CHOLESTEROL.>>ONE DAY YOU WAKE UP AND YOU
SAY I AM ALMOST 50 YEARS OLD, AND I HAVE TO LOOK AT WHERE I
WANT TO BE. I HAVE TO LOSE SOME WEIGHT. I’VE GOT TO CHANGE SOME THINGS
IN MY LIFE. THAT WAS MY WAKE-UP CALL TO SAY
I NEED TO CHANGE SOME THINGS.>>DAVID TRIED LOTS OF DIFFERENT
DIETS OVER THE YEARS. HE WOULD LOSE SOME WEIGHT, ONLY
TO GAIN IT BACK. HE FINALLY BROKE THAT AND HELP
THE CYCLE BY WORKING WITH A SPECIALIST AT METRO HELP THE
WHITE MANAGEMENT CLINIC. THESE DAYS HE TAKES A BREAK AWAY
FROM WORK TO DECOMPRESS AND USES AN ACT ON HIS CAR PHONE TO EAT
HEALTHIER MEALS.>>IT TRACKS ALL THAT FOR ME
AUTOMATICALLY. IT IS VERY IMPORTANT TO RECORD
YOUR FOOD EVERY DAY. IT TRIGGERS THINGS IN YOUR BRAIN
TO STOP EATING AND TO BE MORE IN CONTROL OF YOUR EATING. IT IS REALLY NOT A DIET. IT IS A CHANGE OF LIFESTYLE.>>DAVID HAS BEEN AT THIS FOR
THREE MONTHS AND HAS LOST 35 POUNDS. HE KNOWS HE HAS FURTHER TO GO.>>I DEFINITELY HAVE TO WORK ON
EXERCISE AND FITTING A ROUTINE OF EXERCISE INTO MY LIFESTYLE. IT IS NOT EASY TO ROUTINELY GO
TO THE GYM OR WORKOUT.>>LOOKING BACK NOW, WHAT WERE
YOU DOING WRONG?>>THE BIGGEST THING I DID WRONG
WAS JUST NOT PAY ATTENTION TO WHAT I WAS DOING. YOU GET LOST IN YOUR OTHER
COMMITMENTS AND THE STRESSES THAT YOU GO THROUGH.>>NOW WITH BOTH KIDS IN
COLLEGE, DAVID AND HIS WIFE HAVE MORE TIME TO FOCUS ON
THEMSELVES.>>IT WAS FUN, BUT NOW IT IS
TIME TO DO SOME DIFFERENT STUFF.>>TIME TO TAKE TIME FOR US.>>DAVID STORY IS ALL TOO
FAMILIAR. A JOB, MARRIAGE, KIDS, CARING
FOR A PARENT. IT IS A DELICATE BALANCING ACT. JOINING US NOW IS DR. NOWJANATA. ARE PEOPLE REALLY SELF
MEDICATING WHEN THEY EAT, DUE TO STRESS?>>WE CAN THINK OF THAT IN A
COUPLE OF WAYS. WE USE OFTEN TO REGULATE OUR
STRESS. IT COMFORTS US AND RELEASES
HORMONES THAT CONTRIBUTE TO OUR FEELING BETTER. YOU POINTED OUT IN THE STORY WE
JUST SAW THAT SUGAR, HIGH-FAT, THOSE THINGS BECOME ADDICTIVE
WHEN YOU START EATING THEM.>>BY VIRTUE OF ITS COMFORTING
FACTOR AND THE FACT THAT WHEN WE EAT, WHEN WE EAT HIGH
CARBOHYDRATE FOODS, WE TEND TO FEEL MORE RELAXED. THE SIMPLEST EXPLANATION IS THAT
BLOOD RUSHES TO OUR GUT AND AWAY FROM OUR BRAIN AND WE FEEL
BETTER, BUT WE ALSO KNOW NOW THAT CARBOHYDRATE FOODS TEND TO
BE PRECURSORS TO SEROTONIN THAT HELPS OUR MOOD TO IMPROVE, AND
GRADUALLY, OVER TIME, WE BECOME DEPENDENT ON THAT RELEASE.>>I KNOW THERE ARE DIFFERENT
KINDS OF STRESS. A CHRONIC KIND IS WHAT LEADS TO
OBESITY?>>ONE OF THE CAUSES, CHRONIC
STRESS IS JUST ACUTE STRESS GONE CRAZY. IT IS ESSENTIALLY WHEN NORMAL,
HEALTHY MECHANISMS FOR DEALING WITH STRESS START TO BECOME
TYPICAL. OUR BODY STAYS IN THE KIND OF
RESPONSE WE HAVE IF SOMEONE HITS THE BREAK RIGHT IN FRONT OF US. WE ALSO GO ON HIGH ALERT. THE STRESS RESPONSE IS DESIGNED
TO ALLOW OUR BODY TO DO THAT. IT WORKS WELL IN A STRESSFUL
SITUATION, BUT OVER TIME, IF IT IS CHRONICALLY ACTIVATED, THEN
WE REALLY HAVE A PROBLEM. PEOPLE BELIEVE WE HAVE MORE
STRESS IN OUR LIVES TODAY THAN WE DID A GENERATION BEFORE.>>THERE IS NO QUESTION THAT AND
IN — IN OUR CURRENT CULTURE, PEOPLE WILL ACKNOWLEDGE HIGH
LEVELS OF STRESS AS A FUNCTION OF MANY OF THE SORT OF THINGS
THAT DAVID STORY SUGGESTS TO US.>>I WOULD COME TO YOU AND SAY
HOW DO I MANAGE MY STRESS WITHOUT GOING THROUGH THE MOUTH?>>IT IS A GOOD THING TO START
WITH AN INVENTORY OF STRESS. BEGAN TO TAKE A LOOK AT THE
THINGS THAT ARE STRESSFUL IN YOUR LIFE AND BE THOUGHTFUL AND
PURPOSEFUL ABOUT DEVELOPING STRATEGIES TO DEAL WITH THOSE
DRESSES IN WAYS THAT DON’T LEAD TO THE SORTS OF DELETERIOUS
CONSEQUENCES THAT OVEREATING DOES. YOU MIGHT BEGIN TO THINK ABOUT
ALTERNATIVE EATING — ALTERNATIVES TO EATING, WHETHER
IT IS RECREATIONAL OR DISTRACTION TECHNIQUES ARE
EXERCISE OR MEDITATION. ANY OF THE SORTS OF THINGS WE
MIGHT ORDINARILY UP FRONT SUGGEST MIGHT BE MORE HELPFUL TO
MANAGING STRESS THAN A CHEESEBURGER AND FRIES.>>SO I COULD TRICK MYSELF INTO
NOT EATING.>>YOU CAN TRAIN YOURSELF. I DON’T KNOW ABOUT TRICK
YOURSELF, BECAUSE YOU ARE PROBABLY IN ON THE TRICK. YOU CAN TRAIN YOURSELF IN THE
SENSE THAT YOU ARE TRYING TO REPLACE HAVE WITH THAT LEAD TO
BAD CONSEQUENCES WITH TAVIS THAT ARE HEALTHIER.>>THIS STORY WE SAW WITH DAVID,
IS THAT TYPICAL?>>PEOPLE WILL SAY THEY DON’T
HAVE TIME. THEIR LIVES MOVE SO QUICKLY THAT
THEY DON’T HAVE TIME TO PLAN HEALTHIER MEALS. THEY ARE RUSHING FROM ONE PLACE
TO ANOTHER AND IT’S EASY TO GO THROUGH A DRIVE THROUGH AND PICK
UP SOMETHING THAT SATISFIES A NEED AT THE MOMENT.>>THANKS FOR COMING IN. AFRICAN-AMERICAN WOMEN HAVE THE
HIGHEST RATE OF BEING OVERWEIGHT OR OBESE, MORE THAN ANY OTHER
GROUP IN THE U.S. AFRICAN-AMERICAN WOMEN ARE 70%
MORE LIKELY TO BE OBESE THAN NON-HISPANIC WHITE WOMEN. IT IS NO SURPRISE THAT POVERTY
PLAYS A BIG ROLE IN THIS HEALTH DISPARITY. PEOPLE IN LOW-INCOME
NEIGHBORHOODS ARE MORE LIKELY TO LIVE IN SO-CALLED FOOD DESERTS’,
NEVER WITH LIMITED ACCESS TO LARGE GROCERY STORE SELLING
FRESH PRODUCE. THERE IS A PROLIFERATION OF FAST
FOOD RESTAURANTS AND CORNER STORES WITH LOTS OF CHEAP JUNK
FOOD. 25% OF CUYAHOGA COUNTY RABBIT
RIDGE RESIDENTS LIVE IN TO DESERT. IN OUR NEXT DOOR, WE MEET NIKKI
COLLINS, WHO GIVES US A TASTE OF WHAT IT’S LIKE TO BE POOR, LIVE
NEAR A FOOD DESERT, AND AGAINST THOSE ODDS, TRY TO FEED HER
LARGE FAMILY HEALTHY MEALS.>>I CONSIDER MYSELF TO BE A
HOMEMAKER, AND THIS IS MY JOB AS A PARENT TO RAISE MY KIDS. I TRY TO KEEP MY KITCHEN CLEAN,
KEEP THE DISHES TOGETHER. THEY COME IN, IT IS A HUSTLE
BUSTLE TRYING TO GET THEM A SNACK.>>NIKKI KAHN AND HER HUSBAND,
AND THEIR SIX CHILDREN LIVE IN A PUBLIC HOUSING APARTMENT COMPLEX
ON CLEVELAND’S NEAR EAST SIDE.>>I M ON WELFARE. WE SURVIVE ON FOOD STAMPS. USUALLY I TRY TO KEEP UP WITH
ALL THE MEALS I WANT TO MAKE. LIKE TODAY IS THE 15TH. I COUNT DOWN BECAUSE I GET MY
STAMPS IN THE NEXT MONTH, HOW MANY DAYS I HAVE. IT REALLY IS HEARTBREAKING
BECAUSE OF ME TRYING TO LOOK LIKE AT THE END OF THE MONTH, MY
KIDS, THEY ARE HUNGRY. THEY WANT A SNACK, BUT I DON’T
HAVE THAT ALL THE TIME, AND IT REALLY HURTS MY FEELINGS NOT TO
HAVE THAT ALL THE TIME, BECAUSE THEY’LL BE HUNGRY AND THEY NEED
THAT ENERGY. BUT I STILL STRETCH IT OUT TO
THE POINT WHERE ALL OF US CAN EAT. I STILL HAVE TO GO TO FOOD
CENTERS SOMETIMES TO FILL UP MY PANTRY. USUALLY WHEN I MAKE A MEAL, I
WILL MINIMIZE MY MEAT, BUT EVERYBODY GETS A PIECE. A LITTLE BIT OF STEW BEEF CAN GO
ALONG WAY. WE CAN EAT ALL THIS RICE IN ONE
MONTH. THIS IS A 10 POUND BAG AND THIS
IS WHAT I BUY EVERY MONTH. THIS IS MY MEAL FILLER.>>THE FAMILY DOESN’T OWN A CAR. ONCE A MONTH, NICKY GETZ A RIDE
TO A LARGE WHOLESALE GROCERY STORE WHERE SHE BUYS THINGS IN
BULK. SHE SAYS SOMETIMES PEOPLE GIVE
HER LOOKS FOR BUYING SO MUCH. THEN THEY WILL LOOK AT MY WEIGHT
AND I WILL SEE THE LOOK ON THEIR FACE AND IT KIND OF MAKES ME
UPSET BECAUSE I KNOW I AM A LITTLE BIT OVERWEIGHT FOR MY
HEIGHT, BUT IT’S LIKE, WELL, YOU DON’T KNOW THE STORY, SO HOW CAN
YOU JUDGE?>>THE TRUTH IS PEOPLE WHO LIVE
IN POOR NEIGHBORHOODS ARE MORE LIKELY TO BE OBESE THAN THOSE
FROM HIGHER INCOME AREAS. ONE REASON STEMS FROM THE FACT
THAT MANY LOW-INCOME NEIGHBORHOODS LACK SAFE,
ACCESSIBLE PLACES TO EXERCISE. FOOD DESERTS’ OR ANOTHER ISSUE. THE TERM REPORTERS B.J. THE TERM
REFERS TO POOR ACCESS TO FRESH AND HEALTHY FOODS. THE CLOSEST PLACE TO EAT IS
OFTEN A FAST FOOD RESTAURANT OR CONVENIENCE STORE.>>WE ARE GOING TO SEE EXACTLY
WHAT ONE OF THESE NEIGHBORHOOD GROCERY STORES HAS INSIDE. WE GOT A LOT OF NOODLES, EASY,
FAST UP TO PREPARE.>>SOME OF THE KIDS AROUND HERE
ARE EATING TV DINNERS, NEW RULES, THEY ARE EATING A WHOLE
LOT OF JUNK FOOD. BUT IT’S STILL A MATTER OF
KNOWLEDGE ON HOW AND WHAT TO EAT. WHAT NUTRITIONAL THINGS CAN WE
BUY THAT ARE CHEAP AND THAT WE CAN USE? SO THE RECIPES YOU ARE LEARNING
IN CLASS TODAY, THEY ARE HELPFUL, TASTY, AND ARE THEY
QUICK? THEY ARE QUICK.>>HERE IS WHERE PAT COMES IN. SHE IS A CHEF AND REGISTERED
DIETITIAN WHO GREW UP IN HARLEM, NEW YORK. SHE KNOWS WHAT IT IS LIKE TO BE
POOR. CHEF PAT IS TEACHING A HEALTHY
COOKING CLASS IN A PILOT PROJECT. IT INVOLVES CASE WESTERN
RESERVE UNIVERSITY, THE CUYAHOGA METROPOLITAN HOUSING
AUTHORITY, AND THE OHIO STATE UNIVERSITY EXTENSION.>>YOU HAVE TO START EDUCATING
PEOPLE, TEACHING THEM TO READ FOOD LABELS, TEACHING THEM THAT
EVEN ON A LIMITED BUDGET, THERE ARE WAYS TO EMBRACE A HEALTHIER
LIFESTYLE. YOU MAKE IT REAL. YOU MAKE IT LIVABLE, WHEN YOU
CAN SMELL IT, TASTE IT, SEE IT, FEEL IT. WE HAVE TO RELEARN AND RETRAIN
OURSELVES FOR A HEALTHIER LIFESTYLE.>>A COUPLE OF YEARS AGO, NIKI
TO GOOD NUTRITION CLASS, AND SHE TRIES TO APPLY WHAT SHE LEARNED.>>I DON’T DO CANNED VEGETABLES
BECAUSE IT HAS A LOT OF SODIUM. FROZEN IS MY BEST BET. THAT’S THE CLOSEST THING TO
FRESH VEGETABLES.>>LIVE REMAINS A STRUGGLE, BUT
NIKI IS TRYING TO DO THE BEST SHE CAN FOR HER KIDS.>>I HOPE THEY GROW UP TO BE
RESPECTABLE, WELL-ROUNDED PEOPLE, LET ALONE WELL
NOURISHED.>>KID KIDS THERE. I AM HERE WITH DR. CARLA
HARWELL. YOU TREAT A LOT OF LOW-INCOME
PATIENTS RIGHT IN THE MIDDLE OF ONE OF THE SO-CALLED FOOD
DESERT.>>ABSOLUTELY. I PRACTICE WITH THE FAIRFAX
COMMUNITY, WHICH IS PRIMARILY AFRICAN-AMERICAN AND LOWER
INCOME INDIVIDUALS.>>WE SAW THAT MANY PEOPLE DO
SUFFER BECAUSE OF THE FOOD DESERT SYSTEM, THE WEIGHT IS PUT
TOGETHER. DOES LEAD TO OBESITY.>>IT DOES. WHEN YOU DON’T HAVE ACCESS TO
THE HEALTHIER FOODS AND THE THINGS THAT CAN MAKE A
DIFFERENCE IN YOUR CHOICES IN TERMS OF HOW YOU PREPARE YOUR
MEALS, THAT IS A PROBLEM, IF YOU DON’T HAVE ACCESS TO THAT.>>IT IS REALLY A STRUGGLE FOR
PEOPLE LIKE NIKI, TRYING TO DO THE RIGHT THING.>>SHE IS MAKING SOME HEALTHY
CHOICES, PEAKING FROZEN VEGETABLES OVER CANNED. THERE ARE LITTLE THINGS YOU CAN
DO, BUT WHEN YOU ARE ON A FIXED INCOME AND YOU ARE IN A FOOD
DESERT, THE CHOICES BECOME HARD.>>WHY IS THEIR CONCENTRATION
OF AREAS WITHOUT FOOD?>>IT IS REALLY SAD. IT IS THAT WHEN YOUR
NEIGHBORHOOD, YOU DON’T HAVE ACCESS TO HELP THE, FRESH FRUIT,
EVEN IN TERMS OF TACKLING OBESITY. IF YOU WANT TO EXERCISE, YOU MAY
LIVE IN AN AREA THAT HAS A HIGH CRIME RATE AND THERE IS NO PLACE
FOR YOU TO WALK, OR THERE IS NO GREENERY. WHEN WE USE THE TERM THROUGH
DESERT, WE ARE TALKING ABOUT ACCESS TO FRESH FRUITS AND
VEGETABLES AND HEALTHIER FOOD CHOICES, BUT WE ALSO HAVE TO
LOOK AT THE BIG PICTURE. YOUR NEIGHBORHOOD ALSO
CONTRIBUTES TO HOW LIKELY YOU ARE TO FEEL SAFE TO EVEN BE ABLE
TO GO OUT AND EXERCISE.>>BECAUSE THEY ARE IN A DESERT
LIKE THIS, THEY MAY SEE MORE OBESITY ARE ROUNDED. DOES IT BECOME A CULTURALLY
ACCESSIBLE — A CULTURALLY ACCEPTABLE SITUATION?>>IF THAT IS ALL YOU SEE AND
YOU ARE TOLD NOT NECESSARILY THAT IT IS OK, BUT IF IT IS SEEN
TO BE OK, THEY WILL BE WHAT THEY SEE.>>YOU MENTIONED THE FACT THAT
IT IS DANGEROUS TO WALK IN SOME AREAS. LACK OF EXERCISE HAS TO BE AS
BIG A CONTRIBUTOR AS THE LACK OF FRUITS AND VEGETABLES.>>ABSOLUTELY. ANY SUCCESSFUL DIET HAS TWO
PARTS TO IT. THE FOOD CHOICES THAT YOU MAKE,
AND ALSO BEING ABLE TO EXERCISE. IT IS TRUE THAT YOU CAN EXERCISE
IN YOUR OWN HOME, BUT IF YOU WANT TO GET OUT AND WALK AND YOU
ARE IN A NEIGHBORHOOD WHERE THAT IS NOT SAFE TO DO, YOU ARE NOT
GOING TO DO IT IF YOU DON’T FEEL SAFE.>>WHAT ARE OTHER THINGS YOU
TELL PEOPLE YOU COME IN CONTACT WITH TO HAVE A SITUATION LIKE
THIS?>>LIKE NIKI WAS DOING, I TRIED
TO TEACH THEM THAT THERE ARE SOME SUBSTITUTIONS AND SOME
CHOICES THAT SHE CAN MAKE. TRY TO STEER AWAY FROM THE FOODS
THAT ARE HIGH IN CARDS AND STARCHES. WHEN YOU ARE ON A FIXED BUDGET,
A 5 POUND BAG OF POTATOES CAN GO A LONG WAY. IT IS OK TO EAT FRENCH FRIES,
HAMBURGERS AND HOTDOGS, BUT EVERYTHING IN MODERATION. WHEN YOU ARE ON A FIXED INCOME
AND YOUR CHOICES ARE NOT WHAT YOU WOULD LIKE THEM TO BE, YOU
JUST HAVE TO TRY TO MAKE THE BEST CHOICES THAT YOU CAN MAKE
WITHIN THE CONSTRAINTS OF WHICH YOU HAVE TO DEAL WITH
MONETARILY.>>WE SAW IN THE STORE THERE ARE
JUST LINES AND LINES OF DORITOS AND OTHER SIMILAR TYPE FOODS.>>THAT IS HARD. I EVEN LOOK AT FAST FOODS. IF YOU CAN GO TO A FAST-FOOD
PLACE AND GET TO BURGERS FOR $2, BUT YOU WANT TO EAT A SALAD, THE
SALAD COSTS $5. SO WHAT ARE YOU GOING TO DO? IT IS NOT JUST THE CONVENIENCE
STORES. IT IS REALLY OUR WHOLE SOCIETY,
THE WHOLE FAST-FOOD INDUSTRY. PLATES AND PORTION SIZES ARE SO
MUCH WATER IN RESTAURANTS. EVERYTHING HAS JUST BEEN BLOWN
UP, AND THAT IS WHAT WE TEND TO EAT. UNFORTUNATELY, I THINK THIS IS A
NATIONAL PROBLEM, NOT JUST AN INDIVIDUAL NEIGHBORHOOD ISSUE. THIS IS EVERYWHERE.>>CLEVELAND IS ONE OF THE
NATIONAL LEADERS IN COMMUNITY GARDENS. IS THAT A GREAT START?>>THAT IS A GREAT START. WHEN YOU CAN ENGAGE THE
COMMUNITY TO BECOME ACTIVE PARTICIPANTS IN TRYING TO GROW
SOME OF THESE HEALTH THE VEGETABLES, SO YOU CAN MAKE
BETTER FOOD CHOICES. WE NEED TO HAVE MORE OF THOSE.>>THANK YOU SO MUCH FOR COMING
IN. IN THIS PROGRAM WE HAVE PULLED
BACK THE MANY LAYERS OF OBESITY TO BETTER HELP YOU UNDERSTAND
THE PROBLEM AND WHAT YOU IN YOUR FAMILY CAN DO TO FIGHT AGAINST
THAT. WE LOOK AT THE SO-CALLED FAT
MAPS TO BETTER UNDERSTAND THE ORIGINS OF THE OBESITY EPIDEMIC. WE HEARD FROM RANDY, HIS
UNFORGETTABLE EXPERIENCE AT CEDAR POINT, AND HOW THE TYPICAL
AMERICAN LIFESTYLE BAD EATING HABITS AND NO EXERCISE CAN SNEAK
UP ON YOU. WE LEARN THAT FAT IS NOT IN THE
EYE OF THE BEHOLDER BUT IN A SIMPLE MATHEMATICAL PERFORMANCE
CALLED BMI. ROSIE STORY REVEALED THAT THE
WAR AGAINST WEIGHT AND SOMETIMES INVOLVE OUR OWN DNA AS
WELL AS LONG-HELD FAMILY TRADITIONS THAT ARE OFTEN HARD
TO BREAK. STRESS IS A MAJOR CONTRIBUTOR
TO THE OBESITY EPIDEMIC. DAVID STORY SHOWED HOW A
PROLONGED STRESS CAN CAUSE YOUR BODY TO UNLEASH A HORMONE THAT
INCREASES YOUR APPETITE. NIKI SHOWED US HOW BEING POOR
AND LIVING YOUR FOOD DESERT MAKES EATING HEALTHY MEALS MORE
CHALLENGING AND COMPLICATED. WE WANT TO THANK ALL THE PEOPLE
WHO SHARED THEIR STORIES AND THE MEDICAL EXPERTS WHO PROVIDED
THEIR DEPTH AND INSIGHTS. ONE FINAL REMINDER TO VISIT OUR

  1. Hey there, have you discovered Slim Body Maximizer? (just google it) You will find out about the crimes we commit against ourselves. With Slim Body Maximizer, you will discover how to lose fat quickly.

  2. If you desire to burn off fat fast, you should search google for Cosmos Fat Loss. That can help you get the body you deserve.

  3. now people are claiming they don't have access to large grocery stores and that is the problem? in america? that is crap. and it has nothing to do with black and white. it has to do with boredom and desire. i use food stamps myself. And i discovered that when i stopped buying fun food like Doritos and cookies and ice cream etc then i had more money to spend on fresh veggies and unprocessed good food. You can't help people by filling them with self pity. get real and teach accountability.

  4. Superb documentary, raised some really good issues.

I'm a personal trainer in London, UK. Obesity is on the increase here in London, but one thing we don't suffer from is 'food deserts'. Even the poorest areas have street markets which sell bowls of fruit and vegetables.

One of the biggest problems is the junk food industry. You can't walk for more than 5 minutes in London without passing a junk food outlet. And the supermarkets are full of high salt/sugar ready meals and fizzy drinks.

  5. I hope they do an update to this and the people. Ive been researching the food deserts and obesity in order to help others.

  6. I think we're going to have to regulate fast/processed foods like cigarettes. People can't be educated/trusted to make the right decisions. Sure people still smoke but it's a stigma and the numbers keep falling – we can't save everyone but smoking is not the problem it used to be. Same with obesity…get the #s down and save many, but not all.

While I acknowledge that tons and tons of foods are loaded with hidden sugars, people have GOT to make better choices until the day comes when the gov't decides to get serious about this. You CAN buy better foods, you don't HAVE to go to McDonald's, you CAN choose to work out instead of watch the idiot box. Actually, you can work out WHILE you watch it. It's actually easier and less time-consuming to plan meals out for the week. It's cheaper to buy frozen chicken breast, salmon etc, oatmeal, plain yogurt, etc in bulk. Yes your diet will be "boring" but you will get used to it and have the peace of mind that you are taking care of yourself. Just STOP for a few minutes, sit down, enjoy the food you have made for yourself.

Also with exercise, it's the key to maintaining any progress you make. The irony is people claim they are too stressed or tired or have no time to exercise. Maintenance requires 150 minutes a week, or 30 min a day, five days a week. You have time to do that. If you want to lose, 90 minutes a day would be ideal. I mean don't sit down in front of the tv or your computer and say you don't have time. Plus once you get used to exercising, it actually is a natural stress reducer and will give you more energy. I know running 4-5 times a week keeps me sane, relaxed, and mellow. Like a reset button. Looking good is a bonus.

You can't expect everyone to make your choices for you – you only get this one chance to take care of your body, it's up to you…and that means you can ALSO take credit when you are in excellent shape and health! When I was growing up in the 80s and 90s, there weren't so many fat kids. If you drive by a school today, it is horrifying to see how overweight/obese they are. Times have changed, rapidly.

  7. I did not exercise and I lost 60 pounds in 6 months. I ate lots of fruits and veggies; I quit white bread and white rice; I ate eggs; I ate greek yogurt; I ate oatmeal; I ate homemade soup with healthy veggies; no junk food with msg, high fructose corn syrup , no additives; no fast food; small portions of food; 1 cup of coffee per day; 1 cup of black tea daily; no packaged of factory made food; willpower; incentives every time I weighed myself weekly with a loss of 2.5 pounds per week; no cereal; lots of beans/legumes; lots of nuts; 4 cookies per month with milk; no restaurants; and ignoring hunger pangs. YOU CAN TOO; if you wish. I went from a size 42 to a 34. Greg.

  8. Niki is a role model and proof that as a responsible person, she can stretch out and provide healthy meals for her family. Scratch cooking goes so much further with basics like our grandparents used to do. Prepping and preparing pre made stuff helps when you are hungry and cuts down on the non nutritional, pre made stuff. Thanks for including an example of how it can be done. Instead of blaming external reasons for why it can’t be done, this is how it can be done for the whole family.

  9. Eating in the car is one of the worst habits to develop and once you start doing it it's really hard to stop. Don't eat in the car! You are more likely to eat junk and to eat mindlessly if you do.

  10. It's shocking to hear adults saying nobody taught them how to eat healthy. It might be because I'm European, but this sounds really crazy

  11. Bariatric surgery is a rip-off. In Australia (and no doubt elsewhere) greedy surgeons coupled with less than desirable rebates from government and health funds means that bariatric surgery is viewed as cosmetic surgery that is unaffordable even for many with private health insurance. With a gap payment of at least $5,000 it is something really only the wealthy can afford. The surgery takes only an hour or so with minimal cost for consumables, particularly for stomach stapling which is one of the procedures. (Acknowledged that gastric banding device costs more), so why the massive cost?
Bariatric surgery is NOT cosmetic surgery. For individuals to lose often half their body weight, it saves ongoing chronic health issues and associated future costs to governments and health funds.
